NON-IDENTIFYING INFORMATION (all persons)

ADOPTED PERSON - It gives a description of your birth family when you were born

BIRTH PARENTS - It gives a description of the family that adopted the child

Some birth mothers - Were led to believe that NON-ID is not available
--it is if handled by the Government

If you were born in Canada and taken to the United States, contact the US Dept. of Immigration and obtain a copy of your Entry Visa. This form should show your full birthname. This is required for all persons taking up permanent residence in the U.S.
