Quebec Non-identifying Information – Birth Relatives
Birth Parents and Siblings may apply for non-identifying information.
Non-identifying Information is a social history profile about the adoptive family. You are entitled to this information under Ontario's Child & Family Services Act. If the adoption was handled through a Children’s Aid Society, you must apply to that agency.
For a list of Ontario CAS offices, click here.
If you don't know which Children’s Aid Society handled your adoption, contact:
Adoption Disclosure Registry
Ministry of Community & Social Services
2 Bloor Street West, 24th floor
Toronto, Ontario M7A 1E9
Tel: 416-327-4730
Fax: 416-327-0573
If the adoption was handled privately, that is, through a doctor or lawyer, apply directly to the ministry (see numbers above). If you do not know who handled the adoption, you may ask a family member, or you can contact the ministry (see above).
Non-indentifying information should contain the following:
- birth details that relate to the adoptee
- care of the adoptee prior to the placement
- date of placement
- date of the adoption order
- description of the adopting family: approx. ages, levels of education, physical characteristics, hobbies, ethnic origin and fields of employment
- name of placement agency and follow up
Please note that most agencies have extensive waiting lists for the provision of non-identifying history, but this information is invaluable when conducting a search, so apply as soon as possible.
